Northern California Residents and Institutions (service area)
You may contact the Braille and Talking Book Library to obtain a print application form, or download the online individual application form – English (PDF), (WORD) or the online institutional application form English (PDF), (WORD).
For Spanish applicants, please use the Online Individual Application Form – Spanish (PDF) and be sure to mail it directly to the California Braille and Talking Book Library in Sacramento and not to the National Library Service in Washington, D.C.
